Taylor Swift’s ‘Patient Zero’ Music Video Is a Cryptic Thriller

A round of applause is in order for the showgirl’s big return. On Friday, Taylor Swift released The Life of a Showgirl: The Encore, a deluxe version with four brand-new tracks, which also marked her return to collaborating with Max Martin and Shellback. Tonight, at the MTV VMAs, she was honored for her directing skills and premiered the star-studded music video for her single “Patient Zero,” starring Dakota Johnson and Colin Farrell. It’s the show’s first time handing out its VMA Artist Director Honors. “Patient Zero” will be Swift’s 16th music video that she has either directed or co-directed. It tells the story of an ex-girlfriend (Swift) haunting her ex-boyfriend (Colin Farrell) and his new girlfriend (Dakota Johnson). Now that the video’s out … what does it all mean? Is this a Verity track? Is Swift a good or evil ghost? Does the video tease anything else in the Swift universe? And is that poison?

What happens in the music video for “Patient Zero”?

“Patient Zero” has many shades of gray. Swift is a ghost of girlfriends past haunting Johnson in the video, sleeping in bed next to her, knocking drinks over to warn her to leave her ominous partner. Her ghost literally holds back Johnson’s hair while she pukes, referencing either Swift’s wild night in Belfast five years ago or that this relationship is making Johnson’s character sick. The clues that Swift’s ghost is leaving for Johnson still aren’t really hitting until Johnson’s character starts feeling the effects of the literal poison being fed to her in her coffee and wine and realizes this man may be bad. Things escalate as the two share a car ride home from a fancy gala and the bad man yells at her for embarrassing him. Swift’s ghost is, of course, laid out on top of the car, keeping a close eye on her ex’s new girlfriend. Flashbacks of Swift with Farrell, once happy, flash by, mirroring the exact same life that Johnson’s enduring. Same highs, same lows. A newspaper clipping reveals that Swift, an heiress, was found dead, leaving her ex to inherit her fortune. The video ends with Johnson visiting a gravesite and leaving a rose for the dead ex-girlfriend of a man she will hopefully leave, too. It’s a dark romantic thriller wrapped up in a fun music video. 

Ahead of the video’s release, Swift shared a video on Instagram Stories from filming in March 2026, where she expresses her dream to premiere this on TV. “Walking through her and seeing the way that Shibo has lit this and the way these sets are and how brilliantly Ethan has done with this production design, I’m kind of rethinking the release of the video,” she says. “Like, I need to figure out a way to get this on TV or something. I’m sort of like, ‘Ethan, Google “When are the VMAs?’’” Manifestation! 

Is anyone else fun in the music video?

Matthew Stafford from the Los Angeles Rams shows up as a guy in a tux, and at one point, Cara Delevingne appears to flirt with Farrell. Hard day at the office! 

Does Swift reference any movies in the video?

Swift pulled from a few melodramatic thrillers throughout the years. One film that came to mind was the 1972 thriller Bluebeard. It’s also about a woman trapped in a big concrete house (castle). “With the Patient Zero video, I got to explore the cinematic and literary genres I love so much,” Swift wrote on X after the video premiered. “I wanted to incorporate elements of gothic ghost stories, true crime, psychological thrillers all in a setting I hadn’t really shot in much. The house was a metaphor for the coldness of a relationship that’s faded and atrophied.” One could also see some Don’t Worry Darling creeping in, referencing a trapped woman who is seemingly losing her wits because of a man. Rebecca and Verity, which stars Johnson, are also about a new, naïve lover entering the home of a bad man with a wife hidden in his house of horrors. And, of course, Fifty Shades of Gray could be an inspiration. Swift’s obviously watched all of Johnson’s catalogue.

What has Swift told us about why she made The Encore?

Her Encore was inspired by her success with Showgirl; she says she got together with her collaborators Max Martin and Shellback to celebrate. Conveniently, a studio was nearby, and inspiration struck the trio. “I hope you love them, because they were born out of pure gratitude for your exuberance for and celebration of the album we made,” Swift said on Instagram ahead of the release.

When it comes to “Patient Zero,” Swift said that the inspiration came from a real conversation with someone who had dated one of her exes. They bonded over how crappy the guy treated them. Swift told Capital FM that it was a mutual experience among her friends to kiki with an ex-in-law. “I was hanging out with friends, and one of my friends told us that she had been reached out to by the new girlfriend of an ex of hers asking for guidance on if she had the same experiences with this guy,” she explained. “Everyone in our group started saying, ‘Oh, I’ve been through that too,’ and telling the stories of when this kind of reachout had happened for them.” After watching the video, we have no confirmation on what relationship she’s referencing, adding to the crypticness.

Did she really tease all this at the Emmys?

During the Emmys’ pretaped Law & Order: SVU sketch, Swift joined the team alongside Mariska Hargitay as a rookie detective helping them solve the mystery of whether she’s going to the awards show. Real ones knew she was at the Chiefs game supporting her husband, but she shared an important clue that made a lot more sense a week later. “Saccharide. Ares. From the vineyard. North or South,” she said directly to the camera. The first and last clues were hints to her newest music-video stars, Colin Farrell (saccharide = his show Sugar) and Dakota Johnson (North or South Dakota). “From the vineyard” resembles the “from the vine” line in her new song “Pink Clouding.” As for Ares, it’s basically confirmed it’s a reference to Ares, the god of war, a Greek statue featured in the music video.
